It is alleged the man was driving a Volvo on Pinjarra Road, near Lakes Road around 8.25pm on Friday when it crashed into the back of a Toyota Prado.
The Volvo then veered off the road, crashed through a fence and landed in a paddock.
The 58-year-old driver of the Volvo received several broken bones and was taken to Royal Perth Hospital.
The 29-year-old female driver of the Toyota and a child received minor injuries and were treated at Peel Health Campus.
